Kupiecka 32 B, 65-001 Zielona Góra, KRS 0000516499) processes the data provided by you in the form to maintain correspondence with you. The data administrator can be contacted via e-mail: info@bioortopeda.pl or at the e-mail address given above. The legal basis of the processing is Article 6(1)(a) of the GDPR to deal with correspondence. The categories of persons responsible for data processing are employees and associates appointed for data processing, receiving and creating correspondence, including entities with whom we cooperate in connection with the implementation of our services. Data processing is carried out by Ortholine sp. z o.o. based on your consent, contract or to conclude it, in connection with the legitimate interest of contact with you (Article 6(1)(a), (b) and (f) of the GDPR.) The processing period is based on the duration of the objective and the need to use the content of correspondence. You have the right to access the data, demand its rectification, objection, removal after the period of its processing or limited processing. You also have the right to lodge a complaint to the President of the Office for Data Protection. Payment for an appointment to a bank account

For persons who have not appeared at least twice for an appointment and/or canceled their visit later than 24 hours in advance, the next visits can be booked only after making a non-refundable appointment fee in the amount of PLN 180 to the account No: 91 1140 2004 0000 3402 7527 2986.

In order to guarantee the date of the appointment, it is necessary to pay in advance, so as to send the confirmation of payment not later than 24 hours before the agreed date to the email address: info@bioortopeda.pl.

The title of the transfer should include the name and surname of the patient as well as the date and time of the appointment, determined during the registration.
